Specimen Requirements

Specimen Type: Body fluid, foreign bodies, gross specimens, tissue, etc.

Container/Tube: A tissue/specimen container

Specimen Volume: Entire collection

Collection Instructions:

1. Transport specimen to Pathology Laboratory immediately.

2. Fix tissue in Formalin.

3. Label container with patient’s name (first, last, and middle initial), medical record number (if appropriate), date of birth, and date and time of collection.

Additional Information:

1. The following specimen will be returned to submitting physician:

a. No requisition form

b. Name on requisition does not match name on specimen

2. Indicate medical history and initials of person collecting specimen.

3. Specimen source is required.
